  • "This video traced corporate and technical decisions made before the launching of space shuttle Challenger. NASA and Morton Thiokol, the manufacturers of the Solid Rocket Boosters, knew that the O-rings, the two rubber rings sealing the segments of the boosters were prone to erosion. The metal bowed out on launch and hot gases could escape if the rubber did not seal the joint properly. The protentially dangerous erosion of the O-rings is defined as an acceptable risk. Morton Thiokol engineers were worried of the icy conditions on the day of the launch will make the rubber less flexible. The engineers argued that acceptance and success of previous flights should not be taken as proof of safety. However, NASA management who manage the solid rocket booster decided to launch the challenger as scheduled. The Shuttle Challenger exploded, killing all seven crew member after climbing 10 miles into space because of the erosion of the O-Rings. The Rogers Commission 256 page report descriped the incident as "a failure in management", which is used until today as a model in business schools of how not to make a decision."
