"L'ouvrier d'usine John Q. Archibald et sa femme Denise sont les parents de Michael, un gamin plein d'entrain et de joie de vivre, mais atteint d'une malformation cardiaque. De fait, l'enfant est condamné, à moins de subir une transplantation dont les coûts ne sont pas couverts par les assurances de John. En désespoir de cause, celui-ci se rend à l'hôpital où son fils est soigné et prend en otage un chirurgien et d'autres personnes pour obtenir de force l'opération. Bientôt, l'immeuble est assiégé par les forces policières, les médias et les badauds. John entreprend des négociations avec le policier Frank Grimes, mais sans que les deux hommes en arrivent à un compromis acceptable. Le père éploré en vient alors à vouloir se suicider afin d'offrir son propre coeur à son fils."
