"Foram estudados 22 pacientes com alargamento mediastinal a radiografia de torax com o objetivo de avaliar a utilidade da angiografia radioisotopica (AR) no diagnostico diferencial entre comprometimento vascular e tumoral. Em 10 doentes, os dados radioisotopicos foram confrontados com outros procedimentos considerados diagnosticos em cada caso. Nos outros 12 pacientes, o diagnostico final provavel foi baseado em dados clinicos e radiologicos, influenciados pelo estudo radioisotopico. No primeiro grupo, o alargamento mediastinal correspondia a lesoes tumorais em sete casos e em tres a dilatacoes vasculares. Em nove destes pacientes, a informacao da AR foi confirmada por outros metodos. Em um doente, com diagnostico final de hemangioma de mediastino, a SR sugeria dilatacao localizada da veia cava superior. Em reavaliacao posterior, evidenciou-se que a imagem descrita como dilatacao vascular correspondia a desvio lateral da veia cava provocada pela hemangioma. No segundo grupo, dois pacientes eram portadores de massas mediastinais e 10 de dilatacoes vasculares, e a AR, associada aos dados clinicos e radiologicos, foi considerada indicadora suficiente ao diagnostico, tornando desnecessario o prosseguimento da investigacao. Concluimos que a AR e metodo util na avaliacao de lesoes mediastinais, particularmente quando ha suspeita de etiologia vascular (AU)."@pt
